Application Instructions

Childhood Cancer Survivor Scholarship

Application process:

Part One:
Write a 300-500 word letter describing your experiences in life and your future goals, highlighting why you are applying for this Scholarship.

Part Two: Send us the following:

  • Completed application form of personal information
  • Letter of acceptance from your post secondary education program
  • Statement from your attending physician or family doctor that you have been treated (or are still receiving treatment) for childhood cancer

Submit completed application to:

Selection Process

  1. Applications undergo an initial review to ensure that all the information requested has been submitted and that the required criteria have been met.
  2. Scholarship winners will be selected on the basis of criteria met and numbers of scholarships available

Award Announcements

  1. Scholarship award winners will be notified by telephone.
  2. Unsuccessful applicants will be notified by e-mail.

Teva Canada Childhood Cancer Survivor Scholarship

Application process:

Part One:
Write a personal essay describing the following:
How has your personal journey with cancer inspired you to follow a career in the field of medicine/pharmacy/health sciences? and How will the Teva Canada Scholarship help you in your academic career?

Part Two: Send us the following:

  • Completed application form of personal information
  • Letter of acceptance from your university (pharmacy school, medical school, health-sciences program)
  • A copy of your undergraduate degree certificate (if applicable)
  • Statement from your attending physician or family doctor that you have been treated (or are still receiving treatment) for childhood cancer

  1. Teva Canada Scholarship 2010 Award Winners will be notified by telephone. They will be invited to attend the Childhood Cancer Foundation Annual Meeting in Toronto on September 15, 2010 to receive their award.
  2. Unsuccessful applicants will be notified by e-mail but are encouraged to apply again next year.
